UDUSOK Teaching Hospital Nursing admission form, 2022

This is to inform the general public that the management of the School of Nursing USMANU Danfodiyo University Teaching Hospital, Sokoto admission form is out.

ENTRY REQUIREMENTS

Applicant must possess FIVE CREDIT at not more than two (2) sittings, in the following subjects: English Language, Mathematics, Chemistry, Physics, and Biology from NECO, WAEC or NABTEB

PROCEDURES FOR APPLICATION

1. Visit https://healthrevolve.com.ng/ and select "Usmane Danfodiye University Teaching Hospital Sokoto from the "Remitta Payment Menu.

2. Under "Service select UDUTH School of Nursing". Fill in N5,000.00 under Service amount while filling in all other correct information then click "Generate Payment Instruction.

3. Using the generated Remita (RRR Number), go to any bank to make your payment.

4. Bring generated teller of payment with Remita slip to UDUTH Cashier and collect the receipt

5. Bring the copy of Remita slip and receipt collected from UDUTH Cashier to SCHOOL OF NURSING UDUTH SOKOTO TO GET YOUR SERIAL NUMBER AND PIN

6. Visit http://www.uduth.org.ng/online.form2.php and insert the pin and serial number given to you.

7. Fill your information accurately and submit it after acknowledgment.

8. Please come along with a CLEAR PRINTED COPY of your application form on the date of the entrance exam.
